FORMS & INSTRUCTIONS: Petition for Biology Credit

Follow these step-by-step instructions for filling out your Petition for Biology Credit:

1. You will need a copy of your transcript to fill out your petition and a copy (office or unofficial) must be submitted with the completed petition.

2. Download a Petition for Biology Credit or pick one up at the BioAdvising Office.

3. Enter your personal information at the top of the form. NOTE: Contact information is extremely important in case we need to contact you regarding your course credit.

4. Fill in the first 5 columns with your coursework information from your transcript.

Discipline & Course:	ie BIOL 101, as it appears on your transcript
Grade:	Only a grade A-C is acceptable for transfer to UCSC. If your grade in the course is pending leave this column blank and follow up with your grade information as soon as you receive it.
# Semester Units:	Do NOT convert semester units to quarter units, enter your semester units as they appear on your transcript.
# Quarter Units:	Enter your quarter units as they appear on your transcript.
Course Title:	Enter the title as is appears on your transcript.
UCSC Equivalent:	Enter any course equivalencies, ie. courses already articulated like those found on assist.org

5. Submit the form and a copy of your transcripts to the BioAdvising Office.
